Patient Care Coordinator

Engage pregnant and postpartum patients across risk levels through phone, text, video, and in-person visits to explain care management services and support program enrollment. Coordinate appointments with OB/GYNs, PCPs, pediatricians, and specialists while addressing social determinants of health such as transportation, housing, food insecurity, and childcare. Provide education using approved materials to promote self-management, reinforce preventive care adherence, and document all encounters accurately in the EMR. Collaborate closely with interdisciplinary teams to ensure seamless transitions of care and contribute to quality improvement initiatives focused on maternal health outcomes. This onsite role works in-person within a collaborating OB/GYN office in Hagerstown, Maryland, supporting perinatal and postpartum patients within a mission-driven organization dedicated to maternal health equity.
