PATIENT CARE COORDINATOR

Job Summary
Coordinate front office operations and serve as the primary point of contact for patients throughout their care journey. responsibilities include scheduling and rescheduling appointments, coordinating referrals, authorizations, imaging and therapy appointments, managing patient demographics and insurance information, collecting outstanding financial responsibilities, maintaining accurate EMR data, and serving as a liaison between patients, providers, and ancillary departments. Provide exceptional customer service, ensure HIPAA compliance, facilitate communications, support clinic operations and patient throughput, and contribute to process improvement initiatives. Strong emphasis on patient experience, organization, attention to detail, and proficiency with EMR/EHR systems (Athena a plus).
Required Qualifications
- Education: High school diploma or GED required
- Associate degree or healthcare-related education preferred
- Minimum of 1-2 years of customer service experience required
- Experience with EHR utilization; experience with Athena a plus
- Proficiency with Microsoft Office and electronic medical record systems
- Ability to work independently and collaboratively within a team
- Excellent verbal and written communication skills
- Spanish speaking fluency is welcomed but not required
