Job Summary
Coordinate patient care across the care team, manage referrals and Plan of Care communications, conduct patient outreach via telephone or text, support clinic workflow, track outcomes, and maintain data using SEARHC standards; requires 2 years of healthcare, behavioral health, dental, or relevant administrative experience and proficiency with computers, EHR systems, and MS Office; education lines include an Associate’s degree or 2 years of college (or a Bachelor’s degree in lieu of experience); Basic Life Support preferred; travel not required.
Required Qualifications
- Associate’s degree or 2 years of college in social work, social services, healthcare administration, business administration, or related field OR 2 years of relevant professional experience may be exchanged for a degree; Bachelor’s degree in a relevant field may be exchanged for all required experience; Basic Life Support preferred.
