Patient Care Coordinator (San Diego)

Coordinate patient care by processing orders, arranging services, and obtaining reports to ensure timely delivery for SCAN members. Manage appointments, transportation, and vendor documentation while conducting home visits for assessments and supply delivery. Triage incoming communications, prepare forms for clinicians, and maintain clinical supply inventory. Support clinicians by facilitating virtual visits and proposing improvements based on patient feedback. Work remotely and in the San Diego field area approximately 25% of the time.
