Job Summary
Coordinate care for SCAN members by processing orders, arranging services, and obtaining results from hospitals and labs. Manage patient appointments, transportation, and insurance authorizations while conducting home visits for assessments and supply delivery. Triage incoming communications, prepare forms for clinicians, and maintain inventory of clinical supplies. Support clinicians by delivering essential supplies, facilitating virtual visits, and proposing solutions based on patient feedback. Work remotely with 25% field presence in Riverside, CA, reporting to general supervision.
Required Qualifications
- High School Diploma or equivalent experience
- 1+ years of related experience
- Tuberculosis screening proof or negative screening within the last year
- Background check
Desired Qualifications
- Associate's degree or equivalent experience
- Training or certification as a Medical Assistant
- Bilingual Spanish proficiency
