Job Summary
Complete individualized patient care plans and perform care management services using the electronic medical record system. Maintain frequent contact with patients to provide coordination, support, and manage compliance with care programs to increase positive outcomes. Document all client communications accurately and communicate with Primary Care Clinicians regarding emergent needs or life-threatening episodes. Keep Team Supervisors informed of all issues pertinent to the care plan process and escalate client issues as needed. Demonstrate ability to work with cross-organizational areas to meet patient, family, and partner facility needs while adhering to documentation protocols and best practices for daily work logs. Utilize office equipment including computers, telephones, and copy machines as necessary.
Required Qualifications
- Must be comfortable with speaking on the phone for large amounts of the day
- Must be compassionate and empathetic towards our patients, always demonstrating exceptional customer service
- Ability to take accurate notes to document each task in a timely manner
- Ability to multitask between different patients and workstreams while remaining organized and efficient with time
- Ability to thrive in a fast-paced environment
- Must be able to work full-time, Monday through Friday, 8:30am-5:00pm, on-site in Marlton, New Jersey
- Must be proficient in using a computer, including Outlook and other Microsoft Office programs
- Knowledge of basic healthcare terms, conditions, roles, and basic care principles
- Candidate must be able to pass a drug screen
- Candidate must be able to pass a background check
- Candidate must have a positive attitude
- Candidate must be willing to learn new skills on a continuous basis
