Patient Care Coordinator (Registered Nurse) (RN)

Job Summary
Supervise assigned team members and coordinate daily activities, including interdisciplinary care plans and patient assignments to RNs, LPNs, and CNAs. Complete performance evaluations, maintain census records, and ensure Supervisory Aide visits occur every 14 days. Facilitate interdisciplinary meetings, coordinate student rotations, and manage supply orders while promoting education on hospice care for patients and families. Rotate clinical administrative on-call for 24-hour support and serve as RN in charge during Area Manager absences. Conduct interviews, recommend hiring decisions, and mentor new staff. Maintain strict confidentiality and adhere to all organizational policies.
Required Qualifications
- Graduation from an accredited school of nursing
- Bachelor of Science in Nursing (BSN)
- Licensed as a Registered Nurse (RN) in Arkansas
- Minimum of three years nursing experience in a hospital, home health agency, hospice, or related program
- Valid Arkansas driver's license
- Personal automobile
- Auto insurance
- Knowledge of palliative care with emphasis on symptom control associated with terminal illness
- Working knowledge of oncology
- Knowledge of the holistic approach toward patient care and the nursing process as it applies to hospice patients and families
- Knowledge and acceptance of hospice philosophy and principles of care
- Skill in organizing and prioritizing workloads to meet deadlines
- Ability to recognize both positive and negative aspects of death and grief
- Ability to effectively supervise the work of assigned staff
- Ability to communicate effectively both orally and in writing
- Ability to communicate effectively with co-workers and other customers
- Ability to follow basic safety policies and procedures
- Ability to use good judgment and to maintain confidentiality of information
- Ability to work as a team player
- Ability to demonstrate tact, resourcefulness, patience and dedication
- Ability to accept direction and adhere to policies and procedures
- Ability to work in a fast-paced environment
- Ability to work in a constant state of alertness and concentrate for long periods of time while performing life-threatening procedures, monitoring and/or operating equipment used to perform medical procedures, and/or working with controlled substances and/or medicine
Desired Qualifications
- Bachelor of Science in Nursing (BSN) preferred
- One-year supervisory experience preferred
