Patient Care Coordinator

Screen new patients and caregivers, then onboard them by assisting with documentation, clearance requirements, and orientation training. Staff patient cases with caregivers to ensure cases start promptly while ensuring compliance with legal and company policies, including clock-in and clock-out protocols. Communicate with patients, caregivers, and families to ensure satisfaction and quality service delivery, and assist with on-site visits as needed. No homecare experience necessary; we provide all required training. Bilingual ability is preferred.
