Patient Care Coordinator

Screen new patients and caregivers, then onboard them by assisting with documentation, clearance requirements, and orientation training. Staff patient cases with caregivers to ensure cases are started promptly while maintaining compliance with legal standards and company policy regarding clock-in and clock-out procedures. Communicate with patients, caregivers, and families to ensure satisfaction and quality service delivery, and assist with on-site visits as needed. This role supports our mission to provide exceptional homecare services to patients in Pennsylvania and Delaware.
