Patient Care Coordinator

Screen new patients and caregivers, then onboard them by providing assistance with documentation, clearance requirements, and orientation training. Staff patient cases with caregivers to ensure cases are started promptly while ensuring compliance with legal and company policy, including clock-in and clock-out requirements. Communicate with patients, caregivers, and families to ensure satisfaction and quality service delivery, assisting with on-site visits as needed.
