Patient Care Coordinator (LPN)

The Patient Care Coordinator (PCC) coordinates/assigns field clinicians to patients and works as a partner to RN Case Managers and the business development team to ensure admissions are properly managed. Primary responsibilities include scheduling home health patients with supervision, maintaining the master schedule, updating staff schedules, performing visit verification, identifying discrepancies between visits ordered, scheduled, and documented, coordinating all patient admissions and initial visits, rearranging schedules for call-offs or high utilization, and ensuring timely completion of staff schedules. The role requires collaboration to address patient needs, coordinate with intake and branch operations, and support efficient utilization of services in a full-time, on-site setting.
