Patient Care Coordinator (Kolkata)

Manage Patient Support Programs by liaising with key stakeholders, maintaining accurate patient files, and ensuring timely enrolment of eligible patients. Educate patients to facilitate seamless enrolment and accompany team members on visits to healthcare providers, pharmacies, or support sites. Ensure patients receive medications efficiently, prepare regular performance reports, and maintain electronic records using the Patient Management System. Report adverse events in line with Axios SOPPs and participate in required training. Perform administrative duties, support governance monitoring, and comply with international standards and internal policies. Requires fluency in English and Bengali, a medical or social work degree, and 2 years of healthcare experience. Frequent travel to clients and partners is essential.
