Job Summary
Coordinate patient care with hospitals, discharge planning services, and the program clinical team; conduct facility visits at physicians' request to determine eligibility and assist in processing physician orders; maintain effective communication with program staff and other field resources through weekly meetings; participate in weekly program business development meetings with data, reports, and information about changes in accounts and referral sources; supports development of care plans and resource coordination for patients; RN/LPN/LVN license required; BSN preferred with 3+ years of clinical experience preferred; compensation is a full-time role with a starting pay range of $59,300–$80,900 per year; benefits include medical/dental/vision, 401(k), paid time off, disability, life insurance, and more. Required Qualifications
