Job Summary
Coordinate patient care within the PCMH framework, educate patients about medical home concepts, conduct between-visit outreach for patients with chronic illnesses, assist nursing/Provider teams, support care processes and meetings, supervise staff as needed, connect patients with resources and referrals, support measure-improvement activities, and update the EHR with patient progress. Demonstrates strong communication, confidentiality, teamwork, and computer skills; bilingual ability is a plus; travel between sites is anticipated.
Required Qualifications
- High School Diploma
- at least two (2) years of experience working in community outreach, community resource development or casework participation
- Basic knowledge of computer applications
- Bilingual a plus
- Certified Medical Assistants (CMA) are preferred
