Patient Care Coordinator

Guide patients through care pathways, monitor progress, and proactively address delays while serving as the primary point of contact for families. Optimize scheduling plans to align provider capacity with client needs, manage cancellations and rescheduling, and support contract processes for Schools and Value-Based Care clients. Collaborate with on-site leadership and clinical teams to resolve scheduling concerns, ensure timely case assignments, and maintain cohesive site culture. Utilize Axon, Smart Scheduling, and Salesforce tools to streamline operations and monitor KPIs for data-driven improvements. Requires on-site presence and 4+ years of scheduling experience in ABA services.
