Job Summary
Coordinate resident care and serve as liaison between residents, families, nursing staff, physicians, and community departments to ensure timely, professional service in compliance with regulations. Manage appointments, transportation, admissions/transfers/discharges, and maintain resident records; assist with documentation and HIPAA compliance; greet and assist residents and visitors, and provide information and support to staff. Requires CNA certification, high school diploma/equivalent, and preferred experience with Dementia/Alzheimer’s care; able to communicate effectively in English and handle multiple tasks in a healthcare setting.
