Patient Care Coordinator

Coordinate care from hospitals and skilled nursing facilities in your assigned territory by meeting patients at the bedside to discuss home health services and care plans. Work as part of the local sales team to support operations with resources and information needed for a successful start of care, while assisting Account Executives on sales calls to advance business objectives. This patient-facing role requires an LPN, LVN, or RN license, three years of clinical experience, and familiarity with EMR systems like Watershed and Olio. Travel within your geographical territory is required, and successful candidates will undergo TB screening as part of the program.
