Patient Care Coordinator

Coordinate patient care and scheduling for infertility patients at Atlanta Perimeter Office; serve as a liaison between patients and the clinical team to ensure smooth flow of care and clear understanding of care plans; perform high-volume scheduling in a fully computerized environment; provide compassionate, professional customer service via phone, fax, and email; educate patients about their plan of care, assess needs, triage inquiries, and route to the appropriate team member; greet and guide new patients, prepare informational packets, coordinate with the clinical team to inform patients about their plan of care, and maintain patient charts and records; contribute to a supportive, efficient patient experience and uphold clinic policies and procedures.
