Patient Care Coordinator

Associate Patient Care Coordinator (Float) responsible for patient check-in and order management across multiple Reliant Medical Group locations using Epic. Base location is Southborough, with occasional coverage at Milford, Framingham, and Westborough. Responsibilities include greeting/checking in patients, triaging to appropriate departments, releasing lab and radiology orders in Epic, identifying missing orders, coordinating with clinical departments and Quest staff, preparing imaging CDs for pickup, maintaining accurate patient records, delivering excellent customer service, HIPAA compliance, and traveling between locations as needed. Fast-paced healthcare environment with independent work ~80% of the time and collaboration ~20% with Quest and onsite teams. Ideal candidate is dependable, organized, patient-focused, and capable of managing patient check-ins and order processing across multiple sites.
