Job Summary
Provide direct patient care under the guidance of a Registered Nurse, including basic hygiene, mobility, feeding, hydration, and vital sign monitoring. Act as a liaison between patients and nurses to report changes or concerns, while delivering high-level customer service to patients, families, visitors, and employees. Maintain accurate documentation of patient data, intake/output, and condition changes using professional communication skills. Perform activities of daily living, ambulation, and bowel/bladder elimination care while adhering to infection prevention and safety standards. Work 12-hour afternoons/nights shifts with weekend, holiday, and on-call rotation. Requires BLS certification and recent acute or long-term care experience.
Required Qualifications
- Ability to clearly communicate to other members of the healthcare team
- Ability to understand and follow directions of healthcare team
- Knowledge of patient and healthcare safety standards and regulations (i.e. falls, skin breakdown, suicide prevention, environment of care)
- Infection prevention and transmission based precautions
- Understanding of clerical duties and office based technology
- Team dynamics
- Patient data collection and documentation (i.e. vital signs, intake/output, change in patient condition)
- Basic hygiene and activities of daily living care
- Feeding, hydration, and nutrition
- Bowel and bladder elimination
- Ambulation and patient mobility
- Use of clinical technology
- Compassionate, relationship based approach in care activities
- Accountability for completion of assigned tasks
- Escalation of concerns via chain of command
- Weekend, holiday and on-call rotation required per department policy
- 12 hour shifts
- Afternoons/Nights
Desired Qualifications
- Recent experience in acute care and/or long term care facility (preferred)
- BLS Basic Life Support - American Heart Association (preferred)
- High School Diploma (preferred)
