Job Summary
Room patients, capture health information, take vitals, administer point-of-care testing, and perform age-appropriate screening assessments. Coordinate post-visit care by scheduling specialist appointments, managing referrals, and sharing information with internal and external care teams. Collaborate with providers to monitor patient panels and ensure preventive measures are up to date. Handle front desk inquiries, answer phone calls, respond to emails, and maintain clinical space upkeep. Educate patients on chronic disease management using motivational interviewing techniques and provide anticipatory preventive guidance to families. This role supports a primary care team focused on the Senior Health member population (age 65+), emphasizing human-centered care and empathetic relationships.
Required Qualifications
- At least 1 year of experience in a high touch customer service or patient facing role in a healthcare setting
- Strong written and verbal communication skills
- Experience working on collaborative, diverse and feedback-driven multi-disciplinary teams
- A proven track record of persisting through change, demonstrating a forward thinking perspective when under pressure, and consistently stepping up to take action on challenges
- Proficiency in computer technology such as typing, navigating the internet and using multiple software systems simultaneously
- Must be authorized to work in the U.S.
