Job Summary
Assist clients with Activities of Daily Living including bathing, dressing, grooming, toileting, and meal preparation. Provide medication assistance, housekeeping, laundry, shopping, transportation, and companionship as specified in the client's plan of care. Report daily activity through the agency management system and activity logs, documenting any behavior or conditions out of the ordinary. Practice safety in all activities, maintain confidentiality of records per HIPAA regulations, and report suspected abuse or neglect immediately. Ensure personal appearance and transportation meet company policies while attending requested meetings and in-services. Maintain a valid driver's license, automobile insurance, and "Fit for Duty" physical condition.
Required Qualifications
- 18 Years of Age or Older
- CNA License or alternate experience as a caregiver
- CPR & First Aid Certifications
- Reliable Transportation
- Maintain an established work schedule
- Maintain 'Fit for Duty' physical condition
- Valid driver's license
- Automobile insurance
- Must possess required skills and be able to explain and demonstrate, with or without reasonable accommodations, that the essential functions of the job can be performed
- Work requires good communication skills
- Ability to follow verbal and written assignments
- Demonstrate proficiency with Homecare Company Management System--ClearCare
- Effective interpersonal and communications skills including tact and diplomacy
- Effective organizational and planning skills with attention to detail and follow through
- Establish and maintain effective working relationships
- Maintain confidentiality of work-related information and materials
- Maintain confidentiality of records, correspondence, etc.
- Ensure protection of confidential information that is stored, handled, or maintained by following Company policies and procedures related to confidentiality and/or HIPAA regulations
- Be required to lift or push up to 50 pounds
- Keep your personal appearance in compliance with Avenues Home Care Policies, including abiding by company dress code
- Be timely in reporting for shifts or making scheduling changes
- Report any suspected abuse or neglect immediately up the proper chain of command, as required by law
- Attend meetings and in-services as requested
- Abide by all policies and procedures set forth by the Company
- Work safely and follow safety rules
- Report unsafe working conditions and behavior
Desired Qualifications
- CNA License or alternate experience as a caregiver, preferred
- CPR & First Aid Certifications, preferred
- Prior experience assisting with ADLs or caring for elderly, disabled and other vulnerable clients
