Job Summary
Coordinate the Prospective Payment System and quality reporting program for Medicare patients by reviewing IRF/PAI forms, ensuring supporting documentation, and inputting data into the eRehabData system. Review each form completely, accurately, and timely while monitoring functional status scoring and patient care needs. Collect patient information from medical records, set completion dates for the multi-disciplinary team, and develop policies for CMS guidelines, impairment group coding, and tiering distribution. Act as the primary resource for problem-solving regarding coverage and classification criteria. Requires ASN, Bachelor's degree preferred for nursing, Master's for OT/PT/SLP, and 3-5 years of experience in acute, rehabilitation, or LTACH settings.
Required Qualifications
- Graduate of an accredited OT, PT, SLP, or RN (CRRN preferred) program
- Current licensure in the state in which facility resides
- ASN required
- Bachelor's degree preferred for nursing
- Master's degree required for OT/PT/SLP
- Current Basic Life Support (BLS)/CPR card
- A minimum of three (3) to five (5) years of experience in a patient assessment coordinator role
- Three (3) to five (5) years of experience in the care of patients in an acute hospital, rehabilitation, or LTACH setting
