Patient Advocate

Support newborn families by coordinating appointments, education, and smooth transitions from hospital to home. Assist patients with Oregon Health Plan (OHP) and insurance enrollment while guiding families through the enrollment process. Schedule newborn visits and family tours, coordinate access to pediatric care and community resources. Maintain accurate patient records and ensure HIPAA compliance. Collaborate with clinical and operational teams to create a welcoming, family-centered patient experience.
