Job Summary
Provide chronic disease education and care coordination, develop patient care plans, and assist with self-management goals. Navigate concrete services by making referrals for social determinants of health, facilitating insurance enrollment, and educating patients on low-cost drug programs. Link patients to outside specialists by scheduling appointments, addressing transportation barriers, and acquiring medical records. Complete cancer screening and treatment navigation, including scheduling tests, preparing for appointments, and enrolling eligible patients in the New York State Cancer Services Program. Document all activities in the Electronic Medical Record and conduct screenings using standardized tools. This role requires seamless transitions between desk-based case management and active on-the-floor patient care involving frequent walking and standing. Bilingual English/Spanish fluency, a valid NYS driver's license, and daily car access are required.
Required Qualifications
- Bilingual English/Spanish with written and verbal fluency in both languages
- Valid NYS driver's license and daily access to car
- High School Degree
- Customer service experience
- Passionate about providing high quality care to low income, under-served individuals and those lacking access to health care
- Able to function effectively as a member of an interdisciplinary team and committed to collaborative team based care
- Culturally competent/sensitive (racial, ethnic, linguistic, LGBTQ+, physical ability, etc.) and committed to inclusive, multicultural programming
- Able to understand and follow detailed instructions
- Highly organized and self-directed, able to multi-task
- Flexible and exhibits the ability to adapt as situations evolve
- Comfortable working in a fast-paced, high accountability environment
- Effective in problem solving and exhibits excellent judgment and the ability to prioritize
- Ability to walk and stand for extended periods during daily patient interactions, sitting for prolonged periods while documenting cases, managing records, and participating in telehealth meetings
Desired Qualifications
- College credits, associate's degree, bachelor's degree preferred
- Experience with community and health organizations
