Pathways RN Facilitator PRN

Job Summary
Gather patient condition data, current and future care plans, and identify barriers to optimal care. Document referrals to the Pathways team in the electronic medical record and obtain verbal certification of patient prognosis. Provide resources, education, and support to clients, families, and care partners regarding decision-making. Advocate for and oversee transitions of care from Pathways to necessary services. Communicate with internal colleagues and physicians regarding client eligibility for hospice care and assess needs for resources outside the Integrated Network of Care. Perform this work remotely with occasional on-site meetings in Lakewood Ranch during 8-hour shifts.
Required Qualifications
- Associate's degree (A.A.) or equivalent from two-year college or technical school
- six months to one year related experience and/or training
- equivalent combination of education and experience
- Minimum two (2) years of hospice experience
- Current valid Florida RN license
- Continuing education: As required for licensure
