Job Summary
Care Coordinator coordinates care for Pathways to Success participants, facilitating High-Fidelity Wrap Around or Intensive Care Coordination, developing individualized care plans (IM+CANS), leading Child and Family Team meetings, and coordinating with case managers and other caregivers. Responsibilities include documenting care plans in the electronic health record, educating participants and families on the Wrap Around process, performing community outreach, managing caseloads, and collaborating with multidisciplinary professionals to address health and social determinants. Preferred bilingual (English/Spanish) and must have valid Illinois driver's license for community-based interventions.
Required Qualifications
- Bachelor's degree required, preferably in Human or Health Services or Health Education, or meets the Mental Health Professional designation as determined by the Department of Human Services
