Job Summary
Pastoral Care Practitioner sought to provide confidential spiritual and emotional support to patients and families/carers within Grampians Health Horsham health service. Deliver inclusive, person-centred care that honours beliefs, values, culture and life experiences through conversations, spiritual assessment, care planning, ritual and referrals. Responsibilities include responding to referrals, offering presence, active listening, meaning-making support, grief support, and facilitating culturally/faith-appropriate practices with consent; documenting care activity in the patient record; and contributing to end-of-life and crisis support within scope. Requires relevant qualification or progress toward qualification in theology, pastoral/spiritual care, counselling, or related fields, completion of CPE or equivalent, and IT proficiency for electronic documentation.
Required Qualifications
- Relevant qualification OR substantial progress toward a related qualification (theology, pastoral/spiritual care, counselling, social sciences or psychology)
- Completion of at least one unit of CPE (or recognised equivalent)
- Experience or developing capability in providing spiritual and pastoral care support, including grief and loss, crisis support, end-of-life care, and supportive conversations within scope of practice
- Competence in basic IT and ability to document care in electronic systems with support/training
