Job Summary
Assess client and family spiritual and emotional needs to develop comprehensive care plans, then provide ongoing spiritual and emotional support while liaising with religious leaders. Train team members on pastoral care topics such as grief, loss, and faith, and distribute relevant resources to enhance their skills. Coordinate partnerships through staff meetings and in-services to extend service capabilities, and regularly meet with service managers to report on developments and identify new needs. Visit clients in homes across Northern Sydney to deliver direct care, requiring access to a car with mileage reimbursement. This permanent part-time role involves 15.2 hours per week, 2 days, based at the St Leonards office.
Required Qualifications
- Commitment to engage and align with HammondCare's Christian Mission, Motivation and Mission in Action
- Strong understanding and experience of people living at home with life-limiting illness including dementia, the impact on carers, and journeys of grief and bereavement
- Strong personal and active Christian faith and church involvement
- Theological /Pastoral Care qualifications and experience
- Demonstrated capacity to pastorally care for and enhance the wellbeing of those in need
- Highly developed communication skills including listening, compassion and empathy
- Experience raising up, leading and supporting a team
- Strong administrative and computer skills
- Access to a car
- Australian's driver licence
