Job Summary
Conduct intake assessments for newly enrolled participants and develop individual treatment plans in consultation with instructors, case managers, and guardians. Collaborate with interdisciplinary teams to create case plans, facilitate workshops on employment, academics, and financial literacy, and provide counseling services to young adults and alumni. Maintain accurate case files, input data into Salesforce and the Advance and Earn portal, and write entries for a bi-weekly newsletter. Assist participants in navigating public benefit systems, affordable housing markets, and legal challenges while liaising with funders and community agencies. Work 35 hours weekly at the East Harlem Cornerstone site.
Required Qualifications
- At least three years' experience working with Opportunity Youth
- Bachelor's degree in social work
- Strong writing, speaking, interpersonal and organizational skills
- Demonstrated experience providing case management
- Demonstrated experience facilitating group work
- Demonstrated experience managing data
- Demonstrated experience effectively coordinating entitlements, benefits and community resource networks
- Must value team building
- Must be able to work independently
- Must understand how to actively engage the community to address complex issues
Desired Qualifications
- Knowledge in either database system (Salesforce + Advance and Earn portal)
