Job Summary
Act as a liaison between WelbeHealth’s participants and the PACE care team to onboard participants, coordinate care, and support ongoing participant experience. Responsibilities include maintaining open communication with IDT and external providers, supporting satisfaction surveys and service recovery projects, facilitating communication among participants, families, and caregivers, conducting customer service calls to ensure satisfaction with care appointments, assisting with the grievance process, attending required meetings, and conducting ongoing check-ins to prevent disenrollment. Candidates should demonstrate strong organizational, customer service, and communication skills with seniors and their families, the ability to present information to groups and leadership, adaptability in a rapidly changing environment, and readiness to obtain CPR and First Aid certification within 180 days. Required Qualifications
- High school diploma or equivalency
- Must obtain CPR and First Aid certification within 180 days of hire and maintain certification thereafter; CPR and First Aid certification upon hire preferred
- Bachelor’s degree in communications or relevant field preferred
