Job Summary
Guide seniors and families through enrollment into the program; conduct in-home and community-based assessments; coordinateRequired evaluations, documentation, and intake steps; serve as the main point of contact for referrals and guide participants through each enrollment stage; maintain accurate CRM/EMR records; collaborate with the Medicaid Enrollment Specialist to support smooth Medicaid processing; participate in team meetings, training, and quality improvement; track enrollment progress and navigate barriers to enrollment; contribute to ongoing projects and learn within a mentorship-driven environment.
Required Qualifications
- Bachelor's degree preferred
- Experience working with frail or elderly populations is a plus but not required
- Knowledge of Medicare and Medicaid regulations is helpful
- Experience with community outreach, enrollment, sales, or marketing is a plus
