Job Summary
Maintain individual participant records, daily activity logs, and program notes as assigned. Observe, report, and document participant status, vital signs, and changes in body functions. Assist with personal hygiene, grooming, and activities of daily living, including basic restorative nursing and emergency response procedures. Support the Program Coordinator in designing new ideas, implementing policies, and ensuring program growth. Facilitate socialization, education, and recreation while managing meal procurement, service, and delivery. Set up, implement, and breakdown daily scheduled activities to maintain a clean, safe, and healthy environment. This role supports the Adult Day Health program in Mashpee, MA, with a Monday through Friday schedule from 7:45 a.m. to 2:45 p.m.
Required Qualifications
- High school diploma or GED
- One year experience working with adults or the chronically disabled
- Ability to work with others as part of a team and communicate effectively
- Demonstrate competence per DPH regulations
- Must be able to successfully complete a MA State CORI check, CPS check and company paid, and pre-employment physical
- Schedule Monday through Friday, 7:45 a.m. – 2:45 p.m.
- 35 hours per week
