Job Summary
Provide individual and family counseling to patients in skilled nursing and long-term care settings while coordinating care with facility teams. Adhere to state and federal clinical guidelines and PGS protocols to deliver comprehensive psychiatric support. Educate staff on holistic, patient-centered care and maintain clear, timely documentation of all interactions. Work autonomously within a Monday–Friday day-shift schedule of eight weekly hours, with no nights, weekends, or on-call requirements. This role supports our mission to expand access to quality mental healthcare for seniors through collaborative, empathetic care.
Required Qualifications
- State licensure as a Psychologist, Licensed Clinical Professional Counselor, or Clinical Social Worker (MD, DC, VA, or PA required)
- Monday–Friday availability during normal business hours
- Ability to stand for 20 minutes and lift up to 15 pounds
