Job Summary
Conduct clinical assessments focused on immediate needs, risk, and stabilization goals for adults transitioning from incarceration or hospitalization. Provide crisis intervention, safety planning, and de-escalation support while helping clients develop practical plans for housing, employment, and healthcare connections. Facilitate warm handoffs to coordinate transitions to ongoing care and collaborate with case managers, peer staff, and community partners. Deliver short-term, trauma-informed, and culturally responsive care in respite, clinic, and community settings to support successful community reintegration.
Required Qualifications
- Graduate degree in social work, psychology, counseling, marriage and family therapy, or a related field
- Current California registration or licensure as an AMFT, ASW, APCC, LMFT, LCSW, LPCC, or Licensed Psychologist
Desired Qualifications
- Experience providing behavioral health services in a community-based, crisis, reentry, substance use, residential, respite, forensic, or other non-traditional setting
- Experience working with adults impacted by incarceration, community violence, housing instability, substance use, or other systemic barriers
- Bilingual Spanish/English
