Part Time Social Worker - Skilled Nursing

Job Summary
Coordinate admissions and discharges for residents by completing administrative functions, maintaining state rosters, and facilitating hospital and agency contacts. Organize social services planning, coordinate financial updates for private pay residents, and complete MDS sections. Ensure all clerical tasks are carried out to completion through collaboration with healthcare team members. Participate in scheduled in-service training and education programs. This active role requires frequent standing, lifting up to 50 lbs, and long hours including nights and weekends. A Bachelor's degree in Social Work and 1-2 years of healthcare experience are desired.
Required Qualifications
- Bachelor's degree in Social Work, or closely related human services field
- 1-2 years related experience in a Healthcare setting
- Two years' experience in a similar social work position
- Two years of long-term care or rehabilitation experience
- Ability to read, write, speak and understand English fluently
- Ability to meet or exceed the company's attendance and punctuality standards
- Ability to use Electronic Records and miscellaneous software and office equipment
- Ability to understand and follow directions as given
- Ability to work with minimal supervision
- This position regularly requires long hours and frequent night and weekend work
Desired Qualifications
