Job Summary
Provide intake support, review referral packets, conduct interviews and tours, and facilitate orientation groups for new residents and parents. Deliver individual, family, and specialty group counseling services while maintaining up-to-date case records in the myEvolve electronic health record. Participate in multidisciplinary treatment team conferences to formulate treatment plans, conduct psychosocial assessments, and facilitate family visiting and safety plans. Perform casework services including court reviews, home visits, and acting as a system liaison. Coordinate family engagement activities and work cooperatively with interdisciplinary team members to ensure service provision. Complete additional duties as assigned by the Supervisor. This temporary, part-time leave replacement position lasts approximately 6-7 weeks.
Required Qualifications
- MSW Degree in Social Work
- LMSW
- Valid NY State driver's license
- Acceptable clearance checks for State Central Register
- Fingerprinting as required by the program
- Compliance with health regulations for physical/mantoux testing
- Attending TCI class for certification/re-certification
- Passing a test upon completion of course
- Authorization as agency driver
