Part Time - Social Worker

Part-time Geriatric IOP Therapist provides group, individual, and family therapy to geriatric patients in an outpatient setting within the Geriatric Intensive Outpatient Program. Responsibilities include facilitating evidence-based group therapy for geriatric patients, conducting psychosocial assessments, developing and updating individualized treatment plans, documenting services in the electronic health record, coordinating care with internal and external providers, assisting with discharge planning and referrals, and providing crisis intervention as needed. Requires a Master’s degree in Social Work, Counseling, Psychology, or related field and current state licensure (LCSW, LPC, LMFT or equivalent). Strong organizational, communication, and documentation skills and ability to work within an interdisciplinary team are essential.
