Job Summary
As the supervising OT at Cherrelyn Healthcare Center, evaluate new admissions, develop individualized treatment plans, and lead OT services focused on ADL retraining, cognitive-functional rehab, and return-to-home planning. Conduct initial evaluations, establish resident-centered goals, supervise COTA staff to ensure plan-of-care fidelity, and collaborate with PT, SLP, nursing, and social services on interdisciplinary care planning. Educate residents and families on goals, progress, home strategies, and discharge planning; monitor progress and modify treatment plans accordingly. This role supports part-time and full-time schedules within a SNF environment, with benefits including health insurance, PTO/holidays, retirement plan, tuition reimbursement, and opportunities for clinical leadership growth. Degree from an accredited OT program (MOT or OTD preferred) and licensure as an OT are required or strongly preferred, with SNF/post-acute experience encouraged.
