Job Summary
RN Case Manager in home health and hospice services for Soda Springs, Preston, and Montpelier areas. Responsibilities include completing initial patient and family assessments, developing and maintaining a comprehensive plan of care with goals based on nursing diagnosis, administering medications and treatments as prescribed, counseling patients and families on nursing and related needs, identifying discharge planning requirements, coordinating patient care as assigned by Clinical Supervisor, maintaining documentation per procedures, and participating in on-call duties. Requires Idaho RN license and valid driver's license; travel to patient homes as part of caseload; independent collaboration with physicians and community health professionals; focus on patient-centered, life-changing care.
Required Qualifications
- RN licensed in the State of Idaho
- Valid Driver's License
- Home Health and Hospice experience preferred
