Part Time RN Care Manager-Weekend

Facilitate the discharge planning process from admission to discharge in collaboration with the healthcare team, coordinating patient throughput and overseeing quality patient care, safety, and financial components. Intervene with patients who have complex or high-risk psychosocial needs as assigned while promoting integration of a seamless care model. Maintain strong verbal and written communication skills to encourage healthcare team collaboration and coordinate as evidenced by metrics. This part-time role supports the Care Management model's mission to optimize performance and adoption of best practices. Candidates must hold a current nursing license, a Bachelor's degree in Nursing, and two to three years of acute care experience.
