Job Summary
Facilitate the discharge planning process from admission to discharge in collaboration with the healthcare team. Intervene with patients who have complex or high-risk psychosocial needs as assigned while overseeing quality patient care, patient safety, and financial components. Coordinate patient throughput and encourage healthcare team collaboration and coordination as evidenced by metrics. Optimize performance and adoption of best practices consistent with the Mercy mission, values, and Service Standards. Requires a Bachelor's Degree in Nursing, current state license, and 2-3 years of acute care hospital experience. Weekend part-time schedule available.
Required Qualifications
- Graduate of an accredited School of Nursing
- Current License in the state of employment
- Bachelor's Degree of Science in Nursing
- BLS (CPR) at hire date, Minimum Required, or within 90 days of hire
Desired Qualifications
- 2-3 years acute care hospital setting
- Care Management or Utilization Management experience
- Certification in Case Management
- Knowledge of InterQual or Milliman Care Guidelines
