Job Summary
Part-time SMI Rehabilitation and Discharge Planner responsible for coordinating rehabilitation-related services for adults with serious mental illness and providing case management and discharge planning. Works with RSA VR Counselors and providers to implement vocational/rehabilitation services, conducts assessments to support living, learning, working, and social goals, maintains documentation and data reports, contributes to discharge planning for inpatient and outpatient settings, and delivers staff training and updates on community resources. Requires strong clinical skills, knowledge of psychiatric rehabilitation, excellent organizational and communication abilities, and proficiency with Microsoft Office; includes frequent community travel and office work in EMPACT locations.
Required Qualifications
- Bachelor’s degree in Psychology, Social Work, Family Studies, or other behavioral health related field
- Master's degree preferred
- Minimum of 2 years of case management or inpatient experience in behavioral health
- Must have, or have, the ability to obtain a Level 1 fingerprint clearance card
