Job Summary
Conduct initial and ongoing in-home nursing assessments, develop and update individualized Plans of Care, and provide direct skilled nursing services to clients across Bergen County. Monitor client conditions, coordinate services with physicians and caregivers, and supervise Certified Home Health Aides through regular visits. Complete accurate clinical documentation, provide education to families, and report incidents or changes in status while adhering to infection-control and regulatory standards. This part-time role offers flexible scheduling, competitive compensation, and free training through the QPath Career Pathways Program.
Required Qualifications
- Active New Jersey Registered Nurse license - In good standing
- Current CPR certification
- Current negative TB test or chest X-ray
- Ability to complete all required background checks and screenings
- Valid driver's license and reliable transportation
- Ability to travel throughout Bergen County
Desired Qualifications
- Home care, home health, geriatric, rehabilitation, or community nursing experience
- Experience supervising Certified Home Health Aides
- Knowledge of medication management, infection control, documentation, and care planning
- Excellent communication, organization, and customer-service skills
- Compassionate, dependable, professional, and detail-oriented
- Ability to work independently
- Comfortable using technology for scheduling and clinical documentation
