Job Summary
Administer medications, perform wound care, and execute doctor's orders while conducting client assessments and monitoring conditions. Develop individualized care plans, document OASIS data accurately, and report changes to the Nurse Manager. Collaborate with therapists, social workers, and physicians to coordinate care and manage home health aides or CNAs. Maintain compliance with federal and state regulations while delivering one-on-one care in patients' homes. Requires a Connecticut RN license, two years of home health experience, and OASIS proficiency. Weekend rotation of one weekend per month or less is required.
Required Qualifications
- Minimal of two years of previous Home Health experience
- Must be available on Mondays and Fridays
- A current license as a Registered Nurse in Connecticut
- OASIS experience required
- Have reliable transportation
- Willing to commute to provide in-home patient care
- Weekend Rotation will be required
- Typically, this is one weekend per month or less
