Part Time PRN Home Health Registered Nurse

Conduct comprehensive in-home assessments to evaluate physical, emotional, and environmental needs, translating findings into patient-centered care plans. Deliver individualized, one-on-one nursing care including wound care, infusions, medication management, and chronic disease management tailored to patient goals. Educate and empower patients and caregivers through clear instruction, while monitoring progress closely to adjust plans and prevent hospitalizations. Coordinate care across providers to ensure continuity and complete timely, accurate documentation supporting quality outcomes. Serve as the primary clinical partner for patients and families in the Eau Claire branch on a Monday-Friday part-time schedule with shared on-call rotation. Requires an active RN license with one year of clinical experience, CPR certification, and a current driver's license.
