Job Summary
Deliver physical therapy services per physician orders in coordination with the physician’s Plan of Treatment. Collaborate with healthcare teams, parents/guardians, and nurses; provide written evaluations detailing needs, goals, progress, and outcomes; supervise PTA/HHA as appropriate; complete OASIS assessments where permitted by state; travel as needed and maintain confidentiality and professional boundaries. Strong organization, communication, and problem-solving skills; experience in acute care/rehab and pediatric settings preferred. Daytime PRN schedule, Monday–Friday, in-person at Melbourne, FL with travel as required.
Required Qualifications
- Bachelor of Science in Physical Therapy from an APTA-approved program
- Current, unrestricted state license as a Physical Therapist in the state of practice
- Current CPR certification
- Proven proficiency in clinical assessments, documentation, and compliance with policies and procedures
