Job Summary
Conduct outpatient orthopedic physical therapy sessions in a multidisciplinary medical practice with established administrative and front-desk support. Deliver care to patients covered by Medicare and major New York commercial insurance plans while maintaining consistent scheduled hours. Provide documentation and communication services within a supportive team environment. Tuesday, 3:00–7:00 PM in Lindenhurst; Thursday, 3:00–7:00 PM in Lindenhurst; Friday morning in Hempstead; optional Saturday hours in Lindenhurst. Flexible to accommodate slight schedule adjustments. Requires an active, unrestricted New York State Physical Therapist license, current individual NPI, and CAQH profile. Outpatient orthopedic experience preferred.

Desired Qualifications
- Outpatient orthopedic experience preferred
- No-Fault and Workers' Compensation experience is helpful but not required
- Current participation with Medicare and major New York commercial insurance plans is strongly preferred
