Job Summary
Part-time Home Health Physical Therapist responsible for comprehensive patient evaluations, developing and implementing individualized plans of care, and delivering skilled PT interventions in patients' homes. Key duties include gait, transfer, strength and balance training; ROM and endurance activities; patient/family instruction on safe exercise techniques and device use; collaboration with an interdisciplinary team; documentation compliant with Medicare/CMS and PDGM; and ensuring safe discharge planning. Requires CA PT license and CPR; DPT preferred. Schedule includes 3 days per week in the San Francisco area with per diem pay.
Required Qualifications
- Graduate of an accredited Physical Therapy program; Doctor of Physical Therapy (DPT) preferred
- Active California Physical Therapy license required
- Current CPR certification
- Minimum of one year of experience in a healthcare setting (home health experience preferred)
