Job Summary
Conduct wound assessments and provide personalized treatment plans for patients in skilled nursing, long-term care, and rehabilitation facilities. Collaborate with healthcare professionals to ensure optimal patient care, while completing documentation accurately and educating families on wound care, prevention, and management. Monitor patient progress and adjust treatment plans as needed. Requires an active license, minimum one year of wound care experience, and strong critical thinking skills.
